The doctors welcome new patients who live within our practice area.
All new patients complete a health history questionnaire and are offered a health check within three months of registration.
Medical treatment is available from the date of registration. You will be asked to provide photo identification or two forms of written identification to confirm your address when registering. Please contact reception for further information.

If you do not live within the practice's area and work locally, or have other reasons why being registered at Holsworthy is better for you, then we do operate the Out Of Area Registration scheme for a small number of patients. To find out more about this, and see if you are eligible to apply, contact the surgery. This process can take up to a month so please make sure you have a surgery covering your healthcare whilst this happens.
The practice has the facility to offer all patients having a consulation translation services using Language Line. To respect patient confidentiality we would ask that patients request this themselves at the start of their consultation or before if practical, rather than bringing in their own translators.

When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You will also be asked for photo identification or two forms of written identification to confirm your adddress.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

If you are a visitor to the area, a student returning home for a holiday period, or someone working nearby and find that you need treatment urgently for illness or injury then you may be able to register with the practice as a TEMPORARY RESIDENT. This will allow you access to GP services for up to 15 days or up to three months in some circumstances.
People who are unwell should be advised to call the medical centre as soon as they feel they may need to be seen, with details of their home address, telephone number and normal GP practice as well as their current, temporary address and any mobile number. They will be asked to provide two forms of identification (if possible) and attend the surgery at least 10 minutes prior to their appointment to get all details confirmed and registered.
Visitors living overseas who have a British Passport and are planning to stay in the UK for more than three months are entitled to free NHS treatment and care. If staying less than three months, private fees may apply. Please ask at reception if you wish to be advised.
